Virginia Advance Directives
Although many people find Advance Directive facilitation to be helpful, you are not required to work with a facilitator. If you feel comfortable doing so, you may complete an AD on your own.
Using a pre-made form
If you are completing an Advance Directive without the help of a facilitator, it may be helpful to use a form as a template for your AD. Using an AD form can help you make sure that you include all of the information and instructions that you might want your doctors to have during an emergency.
Not using a pre-made form
There is no particular form that is required in order to complete an Advance Directive. You can create your own form, as long as it meets Virginia’s requirements for an AD (See here for details).
